Contact your local NDIS Office or local NDIS partner or call 1800 800 110 and ask for an NDIS Access Request Form to be mailed or emailed to you.
Download and complete the NDIS Access Request Form.
You can email your completed form, with supporting information to enquiries@ndis.gov.au or Mail hard copies to PO Box 700, Canberra, ACT 2601.
If you have any questions and want to speak with members of the NDIS directly, call 1800 800 110.
Once the NDIA makes a decision about your eligibility to access the NDIS they will send you a letter notifying you of the outcome.
There is a Participant Service Guarantee that outlines how long the decision should take.
If you are eligible to receive funding through the NDIS, the next steps will be to have a planning meeting. The NDIA will contact you to arrange this.
If you are found to be ineligible you can still access services in your community and your Local Area Coordinator can help you with those.

If your situations change in the future, you can make a new access request to be re-assessed.
