Transition Participants to HCA Disability Services
If you’re closing your business, exiting a location or stepping away from particular supports, Healthcare Australia may be able to support affected participants. We provide one-to-one disability support, including personal care, daily living and community access, with a dedicated Customer Care Manager to coordinate their HCA services.
Transition Particular Supports
You don’t need to transition every participant or service. Healthcare Australia can take on particular supports while you continue delivering others. Participants can also continue with their existing Support Coordinator, Plan Manager and other providers.
